Check the diving medical form before you travel, not on the morning of your course or trip. If any answer means a doctor should review the form, sort that before booking final diving dates.

You do not automatically need a medical certificate to scuba dive in Thailand, but every diver should complete the diving medical questionnaire honestly before training or joining a trip. If the form says you need medical approval, you should get that approval before diving.

This guide explains how the PADI-style scuba medical form works, when a doctor may need to sign it, what Thailand Divers needs from you, and why it is better to check everything before booking your Phuket diving dates. If you are still choosing between course options, start with the New Divers page.

Quick answer

  • No certificate needed for everyone: many divers only need to complete the questionnaire.
  • Doctor approval may be needed: some medical history, symptoms or medications can mean you need a physician’s sign-off.
  • Do not guess: read the questions carefully and answer honestly.
  • Best time to check: before you book flights, hotels or course dates.
  • Thailand Divers page: start with the PADI medical forms page.

What is the scuba diving medical form?

The recreational diving medical form is a screening tool. It helps identify whether a diver should speak to a doctor before scuba diving. It is not there to make life difficult; it is there because diving adds pressure, exertion, breathing equipment and underwater conditions to the normal holiday environment.

For PADI courses and many dive operators, the medical questionnaire is part of the normal safety process. If your answers show that medical approval is required, you should get the form signed by a physician before starting the course or dive trip.

Important medical note

This article is general diving information, not medical advice. Thailand Divers cannot diagnose medical conditions or tell you to ignore a medical questionnaire. If the form tells you to seek medical approval, or if you are unsure, speak to a doctor before diving.

Who may need a doctor’s certificate?

You may need medical approval if the questionnaire highlights a health condition, recent symptom, surgery, regular medication or other factor that could affect diving safety. The form itself is the starting point, not guesswork.

Common areas that should be checked carefully include heart or circulation history, breathing or lung conditions, asthma, chest symptoms, ear or sinus problems, seizures, blackouts, recent surgery, pregnancy, diabetes, some medications, and anything that affects exercise tolerance. This list is not a substitute for the actual medical form.

Why you should check before travelling

The worst time to discover a medical-form problem is after you have arrived in Phuket and paid for a course or trip. If a doctor signature is needed, it may not be possible to arrange it quickly enough, especially if you are travelling with a tight schedule.

Checking early gives you time to ask questions, speak to a doctor if needed, and choose the right diving plan. It also avoids pressure on the instructor, guide and boat team on the day.

What Thailand Divers needs from you

Before booking

  • Read the PADI medical forms page.
  • Tell us if you are unsure about any answer.
  • Do not hide a health issue to avoid delays.
  • Ask early if you may need a doctor signature.

Before diving

  • Bring any required signed medical approval.
  • Tell your instructor or guide if anything has changed.
  • Do not dive if you are unwell on the day.
  • Ask if the planned trip suits your experience and comfort.

Medical form mistakes to avoid

  • Leaving the form until the morning of the trip.
  • Assuming a past certification means no medical check is needed.
  • Guessing the answer instead of reading the question properly.
  • Booking a tight schedule with no time to get doctor approval.
  • Forgetting that new symptoms or medication changes can matter.

FAQ

Do all divers need a medical certificate?

No. Many divers only need to complete the medical questionnaire. A certificate or physician approval is usually needed when the form indicates that one is required.

Can Thailand Divers tell me if I am medically fit to dive?

No. Thailand Divers can help you understand the booking process, but medical fitness decisions need to follow the form and, where required, a doctor.

Should I bring a signed form from home?

If the questionnaire says you need physician approval, it is usually best to arrange that before travelling. It is much easier than trying to solve it at the last minute in Phuket.

What if my health changes after booking?

Tell Thailand Divers before diving. New illness, injury, medication or symptoms can change what is safe, even if everything looked fine when you first booked.
