Medical Statement

Are You Fit To Dive?

Before starting any scuba diving course, all participants are required to complete a medical questionnaire. This form is designed to identify any health conditions that may affect your safety while diving. It includes questions about common concerns such as lung conditions, ear or sinus issues, high blood pressure, heart problems, and more.

All students must complete the official PADI Medical Form before enrolling in a PADI scuba diving course. If you can answer ‘No’ to all the questions, no medical clearance is needed to begin your training. However, if you answer ‘Yes’ to any item, you’ll need approval from a physician before participating.

You can download and print the appropriate medical questionnaire in your language using the links provided below.

As a certified diver, it is your personal responsibility to ensure you are medically fit to dive. If you’ve experienced any health changes or conditions since your initial certification, it’s important to review your current fitness to dive.
